ASCENT BATON ROUGE

Who Can Make a Referral?

Anyone that knows a youth can make a referral. Ascent frequently receives referrals from parents, relatives, schools, teachers, counselors, mental health professionals, and probation officers. Although referred, our services are voluntary so the guardian may choose to accept/decline and will need to participate in an intake call.

Areas We Serve:

You may submit a referral by filling out a referral form below and by clicking  "submit." If you have questions, please contact us or call 225.332.8655.

HOW DO I MAKE A REFERRAL?

NEXT STEPS

After you have submitted a referral, the guardian will be contacted by our referral coordinator within 48 hours.

Ascent Wraparound Services is a part of CSoC (Coordinated Systems of Care). Once in our program, the youth is eligible for other CSoC programs.
For more information on how CSoC works and what services are provided, please follow the link to Magellan’s website.
